We're seeking an experienced Support at Home Care Partner to join a small, supportive team in Ulverstone. This is a full-time locum contract with an inital 6 month duration
n
What You'll Be Doing
n
Managing a caseload of approximately 45 clients
n
Coordinating care plans and providing individualised support
n
Onboarding new referrals and conducting assessments
n
Using MAC (My Aged Care) confidently to manage client journeys
n
Working collaboratively within a close-knit team environment




n
What You'll Bring
n
Solid Case Management experience within aged care, community care, NDIS or home care
n
Strong MAC platform knowledge and Support at Home knowledge
n
Experience onboarding and managing recent clients
n
A client-first approach with excellent communication skills
n
n
Nursing/Clinical background highly regarded -Div 1 RN or EEN
n
n
Location: Ulverstone (office + community) with flexible work options available to reduce travel/office days if coming from
Burnie/Launceston/Devonport
etc - offering 2-3 days per week remote working, with at least 2 days in the office.
n
Start: ASAP
n
Hours: Full-time or 4 dpw
