Uniting AgeWell Burwood East is seeking a dedicated Care Advisor to join our team. This permanent part time role covers three weekdays from 9:00am to 5:00pm, totaling 45 hours per fortnight.

You will manage a caseload of older clients, partnering with families to develop personalised care plans that promote wellbeing and independence. You will coordinate services across health professionals and providers, ensuring safe, home-based support and goal-focused outcomes for clients and their support
