As a Support Worker, you'll play a vital role in enhancing the independence and well-being of our clients. Your responsibilities will include:

- Providing Personalised Support:Deliver high-quality, individualized assistance that empowers older adults to live fulfilling lives.
n
- Creating Connections:Offer companionship and emotional support through engaging conversations and meaningful interactions.
n
- Facilitating Independence:Help clients with personal care and transportation to appointments, ensuring they stay connected and active in their communities.
n
- Implementing Care Plans:Follow and execute individual service, healthcare, and positive behavior support plans, tailoring your approach to each client's unique needs.
n

Who we are looking for
n If you have experience working as aSupport Worker, arequalified with a Minimum Cert III in Aged Care, Community Support or related field, and enjoy variety in your day, you might be exactly the person we're looking for!

n

n
- Minimum Cert IIIQualification in Aged Care, or Individual Support (must have completed aged care components).
n
- Experience working with older adults.
n
- The ability to follow and implement individual service plans, healthcare, and positive behaviour support plans.
n




- A desire to use your skills and experience creating fun and engaging activities that support older adults to engage more consistently with their community and peers.
n
- A valid driver's licence.
n
