Key Responsibilities
n
n
- Support medication safety and quality use of medicines (QUM)
n
- Identify and resolve medication-related problems in collaboration with residents, nursing staff, GPs, and families
n
- Assist with medication reviews and optimisation of medication therapy
n
- Support safe medication management during transitions of care
n
- Contribute to clinical governance activities and quality improvement initiatives in line with the Strengthened Aged Care Quality Standards
n
- Provide accessible, practical medicines advice to residents, carers, and aged care staff
n
- Deliver high-quality dispensing and medication counselling services
n
- Provide clinical pharmacy services and health advice to our community
n
- Support vaccination, prescribing, and professional service programs
n
- Work collaboratively within a progressive, patient-focused pharmacy team
n

About You
n
n
- Current registration as a Pharmacist with AHPRA (no conditions)
n
- Medication Management Review (HMR/RMMR) accreditation highly regarded
n
- Experience in aged care and/or community pharmacy desirable
n
- Strong clinical knowledge, excellent communication skills, and a patient-centred approach
n
- Self-motivated, organised, and confident working across multiple facilities and practice settings
n
