Right at Home Gold Coast North is seeking an experienced and passionate Registered Nurse to join our growing team as a Care Manager RN.

About the Role

n

Reporting to the General Manager, you will provide clinical leadership across our community nursing and aged care services.

n

Your key responsibilities will include:

n

n
- Managing a portfolio of clients and overseeing their clinical care and support.
n
- Conducting comprehensive assessments and developing individualised care plans.
n
- Providing clinical advice, escalation support and mentoring to nurses and care teams.
n
- Leading wound care, including complex assessments, treatment plans and reviews.
n
- Supporting clinical care across wound care, catheter care, continence, diabetes, seizures and medication support.
n




- Coordinating nursing referrals, assessments and clinical services.
n
- Reviewing clinical documentation and supporting compliance with regulatory requirements.
n
- Managing clinical incidents, investigations and improvement actions.
n
- Educating and mentoring team members to strengthen clinical capability.
n
- Building strong relationships with clients, families, healthcare professionals and community stakeholders.
n

About You

n

You are an experienced Registered Nurse and confident clinical leader who enjoys working in community care and supporting others to deliver exceptional outcomes.

n

You will have:

n

n
- Current AHPRA Registered Nurse registration.
n
- Demonstrated clinical leadership and people management experience.
n
- Experience in community nursing, aged care, disability or acute care.
n
- Solid clinical assessment, documentation and care planning skills.
n
- Wound care and complex wound management experience.
n
- Experience mentoring and educating clinicians.
n
- Knowledge of aged care, Home Care Packages and community-based services.
n
- Excellent communication, organisation and stakeholder management skills.
n
- Ability to work autonomously and collaboratively.
n
- Current Police Check, First Aid, CPR and NDIS Worker Screening Check, or willingness to obtain.
n
- Current Australian Driver's Licence and reliable vehicle.
n
