Access Aged Care is redefining GP practice in Residential Aged Care Homes on the Central Coast, supported by telehealth and a multidisciplinary team. The role focuses on proactive, relationship-based care across Gosford and Wyong, with dedicated nursing support to streamline rounds and follow-up care.
