We are seeking a Leisure & Lifestyle Co-ordinatorto work in a part time(32 hours/fortnight)capacity based at Iona Digby Harris Home Nhill.
n ABOUT THE ROLE:
n

- The Leisure and Lifestyle Coordinator is responsible for designing, implementing, andevaluating programs that enhance the wellbeing, engagement, and social connectionof the resident.
n
- Must consider residents' likes, preferences, existing community, and family involvement.
n
- The primary goal of a Leisure and Lifestyle Coordinator is toenrich the lives of residents by creating a vibrant, fulfilling, and supportive community.

KEY SELECTION CRITERIA:
n
Essential :
n
n
- Certificate IV or Diploma of Leisure and Health (or equivalent).
n
- Effective communication and organisational skills.
n
- The ability to work independently and lead a team.
n
- Have a clear understanding of the Aged Care Standards pertaining to residents Leisure and Lifestyle.
n
- Have efficient time management skills devoted to resident's personal lifestyle programs, outings, required documentation and planning.
n
n
Desirable :
n
n
- Have previous experience working in aged care or community services.
n
- Competence in using computer programs including Microsoft Word,
Outlook and electronic resident documentation system and other databases.
n
- Demonstrate flexibility to manage programs, schedules, and resources effectively.
n
n
PRE-REQUISITES:
n
n
- NDIS Worker Screening Clearance.
n
- Working with Children Check.
n
- Fully vaccinated (including influenza) as per current Department of Health Directives.
n
- Australian Driver's Licence.
n
n
KEY RESPONSBILITIES:
n
n
- Plan and coordinate leisure and lifestyle activities tailored to individual and group needs.
n
- Conduct lifestyle assessments and contribute to care plans in collaboration with nursing staff.
n
- Mentor and lead the lifestyle team and volunteers, fostering a positive and collaborative workplace.
n
- Encourage participation in social, recreational and physical activities to promote autonomy and wellbeing.
n
- Maintain accurate records of activities, assessments and resident feedback.
n
- Ensure programs align with health standards, accreditation requirements and organisational policies.
n
- Report any concerns regarding the welfare and deviation of health status of anyresident to the Nurse Unit Manager or delegate.
n
