Assistly is seeking an experienced Level 2 Support Coordinator to join our team in Braybrook, VIC 3019.
This is a contract position supporting NDIS participants to understand and implement their plans, connect with appropriate services, coordinate multiple providers and work towards their individual goals.
The successful candidate will already have practical NDIS Support Coordination experience and be confident managing a participant caseload independently.
Previous experience working as an NDIS Support Coordinator is mandatory.
Key responsibilities
Independently manage a Level 2 Support Coordination caseload
Support participants to understand and implement their NDIS plans
Coordinate allied health professionals, service providers, family members, nominees and mainstream services
Source appropriate providers based on participant needs, preferences and goals
Monitor participant budgets, funding utilisation and service agreements
Identify and resolve service gaps, provider breakdowns and funding concerns
Maintain regular communication with participants and relevant stakeholders
Prepare supporting evidence and documentation for plan reassessments and reviews
Communicate effectively with the NDIA, plan managers and other stakeholders
Maintain accurate, timely and compliant case notes and participant records
Monitor participant progress towards their NDIS goals
Identify and escal… safeguarding, funding or service delivery risks
Attend case conferences and stakeholder meetings where required
Advocate for participants within the scope of Support Coordination
Maintain professional boundaries, confidentiality and participant choice and control
Contribute to continuous improvement and high-quality Support Coordination practices within Assistly
Must be able to demonstrate
Independently manage a standard Level 2 Support Coordination caseload end-to-end
Coordinate allied health, providers, family/nominees and mainstream services
Resolve service gaps, budget issues and provider breakdowns
Prepare plan review evidence and supporting documentation
Communicate effectively with the NDIA
Maintain strong case notes and documentation
Monitor participant funding and service utilisation
Work independently with minimal supervision
What we're looking for
Previous NDIS Support Coordination experience is essential
Strong practical knowledge of the NDIS and Support Coordination
Demonstrated experience independently managing NDIS participants
Strong understanding of NDIS plans, budgets and funded supports
Experience coordinating multiple providers and stakeholders
Excellent organisational and time-management skills
Strong written and verbal communication skills
High-quality case note and documentation skills
Solid problem-solving skills and ability to manage service or funding issues
Ability to work independently while contributing to a team environment
Professional, participant-centred approach
Confidence communicating with participants, families, nominees, providers, allied health professionals, plan managers and the NDIA
Current NDIS Worker Screening Check
Working With Children Check where applicable
Current valid driver's licence and reliable transport
