About the Role

n

n
- Provide outreach nursing care with a team in community settings for people experiencing homelessness or poverty.
n




- Offer preventative care, health education, wound management, medication support, Hep C testing, and social referrals to health and community services.
n
- Case management – Coordinate care and local referrals, link clients with appropriate services, and advocate for their health needs.
n
- Work closely with social workers, community organisations, hospitals and other health professionals.
n
- Participate in education, advocacy, and health promotion to improve access to care.
n

About You

n

n
- Enrolled or Registered Nurse (AHPRA) with experience in primary health, outreach, or community care.
n
- Passionate about social justice and supporting people experiencing disadvantage.
n
- Able to work autonomously and problem‐solve in agile environments.
n
- Current driver's licence and access to a reliable vehicle (travel is required for local outreach work).
n
