ACOP Credentialed Pharmacist – Aged Care | Sydney

Chemist Connect is expanding our aged-care pharmacy services and is seeking an experienced ACOP credentialed pharmacist to join our team.

We work closely with multiple aged-care facilities across Sydney, and we are looking for a pharmacist who is passionate about clinical pharmacy and wants to be embedded within residential aged-care facilities, working alongside residents, nurses, GPs and care teams.

The Opportunity

As an ACOP pharmacist with Chemist Connect, you will have the opportunity to:

- Provide high-quality clinical pharmacy services within aged-care facilities
n
- Conduct medication reviews and optimise medication management
n
- Work collaboratively with GPs, nursing staff and multidisciplinary care teams
n
- Support medication safety and quality use of medicines
n




- Build strong relationships with residents, families and healthcare professionals
n
- Contribute to improved health outcomes for aged-care residents
n
- Work across different areas of Sydney, with opportunities to find locations that suit you
n

What We're Looking For

n

We are seeking a pharmacist who:

n

n
- Is AHPRA registered
n
- Holds ACOP accreditation/credentialing
n
- Has experience or a strong interest in aged-care pharmacy
n
- Is clinically focused and committed to improving medication outcomes
n
- Has excellent communication and interpersonal skills
n
- Enjoys working as part of a multidisciplinary healthcare team
n
- Is organised, professional and confident working independently
n
