We are seeking an experienced and patient-focused Registered or Enrolled Nurse to join our Deception Bay Medical Centre on a part-time basis, specialising in Chronic Disease Management and preventative healthcare.

The Role

n

As our CDM Nurse, you will work closely with GPs, nurses, allied health professionals and patients to coordinate personalised care for people living with chronic health conditions.

n

You will empower patients to take greater ownership of their health through education, goal setting, ongoing monitoring and practical support.

n

Key Responsibilities

n

n
- Prepare GP Management Plans and Team Care Arrangements
n
- Complete patient histories, biometrics and chronic disease assessments
n
- Work with patients to establish practical SMART health goals
n
- Develop action plans and coach patients through sustainable health-behaviour changes
n
- Coordinate referrals to allied health professionals and specialists
n
- Conduct preventative health assessments, including:n n
- Heart health assessments
n
- Diabetes risk assessments
n
- 45–49-year-old health assessments
n
- Over-75 health assessments
n
- Aboriginal and Torres Strait Islander health assessments
n
- Intellectual disability, veteran and refugee health assessments
n

n
- Provide ongoing monitoring and support to eligible patients
n
- Perform clinical services such as ECGs, spirometry, urinalysis and Ankle Brachial Index testing
n




- Collaborate with GPs and multidisciplinary teams to deliver coordinated, holistic care
n
- Identify eligible patients and support proactive patient recalls
n
- Maintain accurate clinical records and ensure services meet Medicare and ForHealth requirements
n
- Support other nurses and non-clinical team members in understanding CDM services
n

About You

n

n
- Current AHPRA registration as a Registered or Enrolled Nurse
n
- Previous Chronic Disease Management experience is highly regarded
n
- Primary healthcare or general practice experience is preferred
n
- Ideally, at least two years' experience delivering direct clinical nursing care
n
- Sound understanding of health promotion and disease-prevention principles
n
- Strong written and verbal communication skills
n
- Ability to motivate patients and support them in taking ownership of their health
n
- Excellent organisation, prioritisation and time-management skills
n
- Proactive, adaptable and confident working independently
n
- Collaborative approach and commitment to delivering quality patient care
