Job Description

Anglicare NT in Australia's Northern Territory is seeking a Moving On Case Manager to provide case management, advocacy, assessment, referral and practical support to young people who have left care.

You will help with housing, education, employment, life skills and community connections while administering brokerage funding and the Transition to Independence Allowance (TILA), delivering trauma-informed, youth-focused services.
