About the Home Care Support Worker role

n

Support consumers to stay independent at home and connected in their community. Provide care, social support and group activities that promote wellbeing and inclusion, while working with a passionate team committed to improving health outcomes for all.Reporting to the Social Support Coordinator, this casual role is based in the Northern Suburbs and supports meaningful, community-focused work.

Your Passion and Commitment will see you:

- Support clients with personal care, domestic tasks and social needs
n
- Plan and run gentle exercise, group activities and outings
n
- Promote independence with respectful, person-centred care
n
- Travel to homes and deliver tailored, in-home support
n
- Build confidence and encourage meaningful client participation
n

n

To succeed in this role you will have:

n

n
- Certificate III in Home and Community Services, Aged Care, Disability or equivalent
n
- Current Victorian Driver's License
n
- Current Light Rigid Endorsed Driver's License (desirable)
n
- Qualifications in Food Safety (desirable)
n
