The Co-Group are hiring an experienced and compassionate Registered Nurse / Case Manager to join our growing team in Kellyville. This is an excellent opportunity for a dedicated healthcare professional who is passionate about supporting older Australians and people living with disability to achieve their goals and maintain independence.
This flexible casual role offers approximately 28 hours per week across 4 days, with the option to work school-friendly hours for the right candidate.
About the Role
As a Registered Nurse / Case Manager, you will play a key role in assessing client needs, developing and reviewing care plans, coordinating supports, and providing ongoing clinical oversight. You will work closely with clients, families, support workers, healthcare professionals, and community stakeholders to ensure high-quality, person-centred care.
You will also contribute to business development activities, community engagement, and the ongoing growth of our services.
Key Responsibilities
- Conduct client assessments and develop person-centred care plans
- Monitor client outcomes and review support regularly
- Coordinate referrals and liaise with service providers and stakeholders
- Provide clinical guidance and oversight for clients requiring nursing support
- Maintain accurate documentation and client records
- Build robust relationships with clients, families, community organisations, and referral partners
- Support business growth through community networking and relationship management
- Contribute to quality improvement and clinical governance initiatives
About You
To be successful in this role, you will have:
- Current AHPRA registration as a Registered Nurse
- Minimum 2 years' experience in Aged Care and/or Disability Services
- Demonstrated case management experience
- Strong knowledge of person-centred care planning and NDIS or Home Care frameworks
- Experience in wound care, continence assessments, and clinical support services (highly regarded)
- Excellent communication and relationship-building skills
- Ability to work autonomously while contributing to a collaborative team environment
- Current Driver's Licence and access to a reliable vehicle
- Current NDIS Worker Screening Check, National Police Check, First Aid and relevant compliance requirements
