About the Role
We are seeking a passionate and experienced Registered Nurse to join our team as a Care Partner in a community-based role across the Perth metro region.
Working Monday to Friday, with participation in an after-hours on-call roster as required, you will be responsible for coordinating and delivering care management activities under the Support at Home program, supporting clients with low, medium, and high care needs.
This is a rewarding, hands-on role where you will build strong relationships with clients, families, and multidisciplinary teams, while delivering safe, effective, and individualised care that promotes independence and wellbeing.
There may also be an element of direct care delivery as part of this role.
Key Responsibilities
- Conduct comprehensive in-home clinical assessments (initial and ongoing)
- Develop, implement, and review person-centred care plans
- Coordinate and manage services under the Support at Home program
- Monitor client wellbeing, clinical needs, and outcomes
- Support budget awareness and ensure services align with funding guidelines
- Provide clinical guidance and support to care staff
- Liaise with GPs, allied health professionals, and community services
- Maintain accurate and compliant clinical documentation
- Conduct workplace health and safety assessments in client homes
- Contribute to continuous improvement and quality initiatives
About You
You are a compassionate and confident Registered Nurse with a strong understanding of aged care and a commitment to person-centred care. You build meaningful relationships and bring a collaborative, supportive approach to your work.
You will also have:
- Current AHPRA registration as a Registered Nurse
- Minimum 3 years’ experience, preferably in aged care or community care
- Experience in care planning, assessments, and case management
- Strong understanding of aged care standards and compliance
- Excellent organisational and time management skills
- Ability to work autonomously with a strong team focus
- Current driver’s licence (Vehicle provided)
- National Police Clearance
- First Aid Certificate
