About the Role:
The Lifestyle Program Coordinator is responsible for planning, coordinating, facilitating and evaluating meaningful lifestyle and wellness programs that enhance the quality of life of residents living in residential aged care.
The role ensures the delivery of engaging and person-centred activities that support residents' physical, social, emotional, cultural, spiritual and cognitive wellbeing. The position works collaboratively with residents, families, volunteers and the broader multidisciplinary team to maximise resident participation and promote a sense of purpose, belonging and connection.
This position does not have direct people management responsibilities but provides day to day guidance and support to team members, students and volunteers involved in lifestyle activities.
In this role, you will be employed on a Permanent Part Time basis at our Nubeena facility.
As Lifestyle Program Coordinator – your duties may include;
- Design, implement, and evaluate diverse and engaging leisure programs that meet residents' needs and preferences.
- Ensure that activities support residents’ social, physical, cognitive, and emotional well-being.
- Develop individualised lifestyle plans in collaboration with residents, families, and clinical teams.
- Organise and oversee special events, outings,
cultural activities, and community engagement initiatives.
- Ensure programs are aligned with Aged Care Quality Standards and the Aged Care Act.
- Work collaboratively with nursing staff, allied health professionals, and volunteers to integrate lifestyle activities into holistic care.
- Communicate effectively with families and significant others to ensure resident needs are met.
You will need to have:
- Proven experience in planning, coordinating, and delivering lifestyle and leisure programs in an aged care or community setting.
- Strong understanding of the social, emotional, and cognitive needs of older adults, including those living with dementia.
- Knowledge of the Aged Care Quality Standards and regulatory requirements.
- Solid communication and interpersonal skills, with a compassionate and resident-focused approach.
- Ability to manage documentation, record-keeping, and compliance requirements.
- Proficiency in using electronic care management systems.
- Certificate IV in Leisure & Health (or equivalent qualification).
- Current First Aid and CPR Certificate (or willingness to obtain).
- Valid registration to work with vulnerable people with NDIS endorsement
- Valid National Police Check
- Current Drivers Licence
