About the Role
Grampians Health Horsham is a 20 bed sub-acute facility that provides admitted rehabilitation, geriatric evaluation and management (GEM), transition and palliative care services within a multidisciplinary team and patient focused setting.
Work Schedule
7‐day, 24‐hour rotating roster including day, afternoon, and night shifts.
Permanent full‐time, part‐time and casual positions available.
Key Responsibilities
Deliver individualised nursing care as part of a multi‐disciplinary team
Liaise and effectively communicate with relevant medical, nursing and allied health team members
Promote evidence‐based practice that has a problem‐solving approach
Encourage patients, carers and families to participate in all stages of care
When appropriate, supervise and direct clinical practice of junior Registered Nurses, Enrolled Nurses, and undergraduate nursing students and provide feedback
Skills & Experience
Registered Nurse with current AHPRA registration
Excellent communication and problem‐solving skills
Knowledge of contemporary nursing theory and practice in relation to the care of sub‐acute patients/clients
Thorough understanding of quality systems and health services accreditation

Additional Information
All appointments are subject to a satisfactory police records check, immunisation clearance and Working with Children check and/or NDIS Worker Screening Check.
All employees must be willing and able to wear Personal Protective Equipment (PPE) when required, which at a minimum is a surgical mask.
Additional PPE requirements are role‐dependent.
