Key Responsibilities
Deliver high-quality, person-centred care in clients’ homes and in our SIL
Support clients with daily living activities including personal care, mobility, and meal preparation
Provide compassionate care for individuals living with dementia and complex needs
Promote independence, dignity, and wellbeing at all times
Observe, document,



and report changes in client condition to the nursing team
Follow care plans and work collaboratively within a nurse-led model
Build trusting relationships with clients and their families
Maintain qualified standards, confidentiality, and compliance with policies and procedures

Desired Qualifications & Skills
Certificate III or IV in Aged Care, Disability, or Individual Support (or currently studying nursing)
First Aid and CPR
Current Manual Handling (or we can provide)
NDIS modules mandatory
Current police check
WWVP
Must have workers screening ID
Medication training for cert 4
Solid understanding of person-centred care principles
Experience in home care and/or dementia support (preferred)
Excellent communication and interpersonal skills
Ability to think critically and respond to changing client needs
Reliable, organised, and able to work independently
Commitment to high standards of care and professionalism
Current driver’s licence and willingness to travel between clients
