In the newly created role of Care Partner, you will be providing relationship-centred care management that delivers exceptionalclientoutcomes,strengthens wellbeing, and supports clients to thriveand livesafely at home.

Achieving billable hours and service delivering expectations through effective care planning, funding utilisation, billing accuracy, and proactive management of client portfolio
n
Undertaking person-centredcare planningin accordance withCatholic Healthcare'smodel of care
n
Educating clients and their families on program guidelines, inclusions and exclusions and possible financial contributions
n




Communicating and coordinating with members of the clinical, allied healthand operational teamstoidentifyclientcare needs, planinterventions, and review outcomes of care
n
Using screening,assessmentand risk dashboards toidentifyhigh riskclients and prioritizetimelyreviews
n
Initiating case conferencing to support hospital, rehab or respite discharge planning forwellbeing interventions, serviceadjustmentsandtimelyreferrals
n
Communicating and coordinating with members of the clinical, allied healthand operational teamstoidentifyclientcare needs, planinterventions, and review outcomes of care
n
Promoting Catholic Healthcare Value proposition and service offerings while enabling clients to live at home independently with choice, control and connecting them to services that are protected,personaland effective
n
Ensuringaccuratebilling,timelydocumentationand alignment of services with client goals and funding availability
n
Understanding and adhering to the Aged Care Standards in all interactions,practicesand documentation
n
