ACCORD CARE is urgently seeking experienced Disability Support Workers to join a Supported Independent Living household in Zillmere.
You will support two participants with complex health needs, including diabetes and mobility support, in a home setting.
Shifts include morning, afternoon and sleepovers.
The role requires compassionate, patient-centred care, accurate documentation and teamwork with coordinators and clinicians to ensure safety and quality outcomes.
