About the Role
As a Care Partner you will manage a portfolio of aged care clients receiving home care services, ensuring they receive high-quality, person-centred support tailored to their goals and needs.
You will work closely with clients, families, support workers, allied health professionals, and internal staff to coordinate outstanding care outcomes.
This is more than a case management role — it is an opportunity to become a trusted partner to clients and families, helping improve lives every day.
Location: Sydney, NSW
Hours: Monday to Friday - Flexible start and finish times available
Key Responsibilities
Conduct client assessments and care planning
Develop, review, and update individual care plans
Coordinate supports and services to meet client goals
Manage client budgets and funding utilisation
Monitor client wellbeing and respond to changing needs
Build trusted relationships with clients and families
Maintain accurate case notes and compliance documentation
Manage incidents, feedback, and complaints
Liaise with health professionals and external stakeholders
Ensure services align with aged care quality standards
About You
You are a compassionate and confident nurse who enjoys building relationships, solving problems, and making a genuine difference.
Essential Requirements
Enrolled Nurse qualification
Current AHPRA registration
Experience in aged care, home care, community nursing or case management
Strong communication and relationship-building skills
Organised with excellent time management
Confident using Microsoft Office and care management systems
Current driver's licence and reliable vehicle
First Aid & CPR Certificate
Current Police Check and Working with Children Check
NDIS Worker Orientation Module and NDIS Screening Check
Full Australian working rights
Highly Regarded
Home Care Package experience
Knowledge of aged care reforms / Support at Home model
Dementia or complex care experience
