About the role

We are seeking reliable and compassionate Disability Support Workers to support participants across South East Queensland, particularly the Logan and Gold Coast areas. You may work across participant homes, community settings and Fortify service environments depending on participant requirements.

As a Disability Support Worker, you will provide protected, respectful and individualised support in accordance with participant support plans, care plans, management plans and behaviour support plans.

You will work collaboratively with case managers, care coordinators, nurses, families, carers and external professionals to deliver consistent, high-quality support. You will also be responsible for completing accurate documentation and promptly escalating any risks, changes or concerns.

Duties & Responsibilities

In this role, you will:

- Provide direct, person-centred assistance in line with participant plans and guidance from the supervising team.
- Support participants with daily routines, community access, appointments, outings and social participation.
- Assist with personal care, domestic tasks, meal preparation and authorised transport where required.
- Encourage participants to build independence, develop skills, exercise choice and work towards their individual goals.
- Support participants to engage in age-appropriate activities, employment-related activities, in-home routines and community access.
- Maintain participant privacy, dignity, choice and professional boundaries.
- Promote dignity of risk while identifying and escalating matters requiring further review.
- Undertake medication-related or high-intensity support tasks only when appropriately trained,



assessed as competent and authorised.
- Follow participant-specific support plans, management plans, care plans, behaviour support plans, delegation requirements
- Complete accurate and objective shift notes, client notes, communication book entries and other required records on the same working day as the shift, unless otherwise directed.
- Provide clear handovers and communicate effectively with participants, families, carers and relevant professionals.
- Report incidents, hazards, medication concerns, restrictive practice concerns or changes in a participant's presentation through the appropriate escalation pathway.
- Participate in supervision, mandatory training, competency assessments, staff meetings and participant-specific training.
- Contribute to safeguarding, incident follow-up, participant reviews and continuous quality improvement.

What you will bring

- A Certificate III in Individual Support, Mental Health, Aged Care, Community Services, Disability Work or another relevant qualification, or equivalent experience.
- A current NDIS Worker Screening Check, or the ability to obtain one.
- A current Blue Card, or the ability to obtain one where required.
- Current First Aid and CPR certification, or the ability to obtain and maintain certification.




- The ability to complete the NDIS Worker Orientation and Client's role-specific induction requirements.
- A current Queensland driver licence and access to a roadworthy, insured vehicle where required for the role.
- The ability to provide all required identity, screening and compliance documentation.
- Availability to work rostered shifts across participant homes and community locations
- Strong written, verbal and interpersonal communication skills.
- The ability to complete timely, accurate and objective documentation.
- A genuine commitment to participant dignity, confidentiality, safeguarding, professional boundaries and the NDIS Code of Conduct.

It would be advantageous if you have:

- Previous experience in disability, mental health, aged care, community services or a similar support role.
- Experience supporting participants with complex needs, behaviour support requirements or high-intensity supports.
- Experience using electronic records, rostering or client management systems.
- Additional training in medication support, positive behaviour support, restrictive practices, manual handling or mealtime management.
