Moreton ATSICHS operates a growing network of healthcare clinics for local Aboriginal and Torres Strait Islander people. Our model of care provides holistic health and wellbeing services to support our Community across their lifespan. We bring together primary care, allied health, dental, mental health and wellbeing, maternity and early childhood, disability services, specialist services, and aged care as one team focussed on clients’ individual needs and goals.

About the role
As Care Coordinator – MATSICHS Social Health, your role is to provide coordination of supports to Aboriginal and Torres Strait Islander clients experiencing complex social and mental health challenges.
You will undertake structured intake, screening, and triage support, provide brief psychosocial support within scope, and work collaboratively with clinical practitioners where higher-level assessment or intervention is required.
Your day to day duties may include:
- Act as a central entry point for referrals to Social Health services.
- Conduct initial intake, screening, and structured assessments (non-clinical).
- Escalate complex or high-risk presentations to Clinical Care Coordinators or senior clinicians.
- Support development of care plans and coordination of services
- Manage a caseload of clients, ensuring follow-up and engagement.
- May be required to work across Caboolture, Morayfield, Margate and/or Strathpine from time to time.
About you

- Current Criminal History Check.
- Current C Class Drivers Licence (QLD).
- Current Working with Children’s Check (Qld Blue Card) or AHPRA Registration.
- Certificate or Diploma in Community Services, Mental Health or similar or willingness to obtain.
- Relevant experience.
