- Provide person-centred support to participants across SIL, in-home, community access, respite and social supports.
- Assist with daily living, personal care, meal preparation and household tasks where required.
- Support participants to access the community, social activities, appointments and recreational activities.
- Encourage independence, choice, dignity and participation.
- Build positive and respectful relationships with participants and their support networks.
- Complete required documentation and follow support plans, safety procedures and workplace policies
- Follow individual support plans and organisational procedures.
- Complete progress notes and required documentation.
- Work collaboratively with families, coordinators and the wider support team.
