Your opportunity
As a Transition Care Program Registered Nurse, you'll provide comprehensive assessment, care coordination and clinical management for older people transitioning from hospital to home.
Working as part of a multidisciplinary team, you'll support clients to achieve their restorative goals, maximise independence and safely connect with ongoing community supports.
You'll manage clients throughout their Transition Care journey, from admission and assessment through to goal monitoring, clinical review and discharge planning. Partnering closely with clients, families, hospitals, GPs and community providers, you'll play a key role in ensuring safe transitions and positive client outcomes
You’ll make an impact by
- Partnering with clients and families to complete comprehensive assessments, develop personalised care plans and establish meaningful restorative goals.
- Coordinating care throughout the Transition Care episode, monitoring progress and supporting clients to achieve greater independence and wellbeing.
- Delivering evidence-based nursing care, including clinical interventions such as wound care, medication management, continence support and chronic disease management.
- Leading discharge planning and coordinating referrals to ensure seamless transitions to community services, Support at Home and other ongoing care pathways.
- Identifying, managing and escalating clinical risks,
while maintaining high standards of documentation, compliance and client safety
What you’ll bring
- Current registration as a Registered Nurse with AHPRA
- Minimum two years' clinical experience
- Demonstrated experience in community nursing, rehabilitation or aged care
- Experience in comprehensive clinical assessment and care planning
- Current driver's licence
- National Police Check

- Strong assessment, care coordination and clinical decision-making skills
- Experience supporting restorative, wellness and reablement approaches to care
- Excellent communication and relationship-building skills with clients, families and stakeholders
- Strong organisational skills with the ability to manage competing priorities and documentation requirements
- A proactive approach to quality improvement and evidence-based practice
- Commitment to helping older people maintain independence, confidence and quality of life
