About the role
Disability Support Worker providing dedicated in-home support during weekdays/weekend shifts (including early mornings/evenings, weekends, and public holidays). You'll be supporting individuals with physical disabilities in their family home environment, specifically Muscular Dystrophy, helping them achieve their goals, build capacity, and live fulfilling lives. This role requires a balance of physical assistance and a respectful, person-centered approach. You will work closely with the participant and our care coordinators to follow a specialized support plan.
Key responsibilities
- Perform secure manual handling and transfers using specialized equipment (e.g., ceiling hoists, floor hoists, slide sheets).
- Assist with mobility, positioning, and community participation using an electric wheelchair.
- Collaborate with the participant's allied health team (Physiotherapists, OTs) to implement daily stretching or therapy routines.
- Providing person-centered support for daily living activities, both at home and in the community
- Supporting physical health and emotional well-being
- Assisting individuals with attending community-based activities and appointments
- Creatively and resourcefully enabling individuals to reach their personal goals
- Completing accurate documentation and reporting: Accurately document shift notes and incident reports via our provider care management platform.
About you
- You are an experienced professional with experience in Muscular Dystrophy who understands NDIS practice standards. You excel at maintaining professional boundaries while delivering warm, dignified care. You are physically fit,
highly punctual, and a proactive communicator.
- Excellent communication skills
- Demonstrated experience in providing quality support to individuals with physical disabilities
- Adaptability and strong teamwork skills
- Ability to manage the physical aspects of the role
- A strong understanding of professional boundaries
- Minimum 6 months of paid experience in Disability, Individual Support, or Mental Health
- Current Manual Handling and Medication Assistance Competency Certificates
- Current QLD "Paid" Blue Card
- Current QLD Yellow Card or NDIS Workers Screening Clearance
- NDIS Worker Orientation Module certificates
- Current First Aid Certificate (issued within the last 3 years)
- Current CPR Certificate (issued within the last 12 months)
- Infection and Prevention and Control Certificate (Hand Hygiene)
- COVID-19 Vaccination/immunisation Record
- Evidence of work rights in Australia
- Current Driver License and access to your own reliable transport
- Current Full Comprehensive Car Insurance
- Smartphone (iOS or Android)
