About the role

Working within the Care team, you’ll collaborate directly with participants, families and stakeholders to deliver high-quality assessments, personalised care planning and comprehensive case management, which promotes participants goals and wellness outcomes.

Key responsibilities:

- Conduct HCP assessments and reviews in participants’ homes

- Convert client needs into funding opportunities to maximise package utilisation

- Build strong relationships with participants, support networks and internal teams

- Deliver high-quality, person-centred care

- Provide comprehensive case management and advocacy

- Collaborate with allied health, nursing partners and others to coordinate multidisciplinary supports

- Manage incidents,



feedback and complaints in line with ACQS requirements

- Record and maintain high quality, comprehensive documentation and case notes

- Travel is required with km reimbursement provided

About you

- Qualifications in case management (e.g. Diploma in Nursing, Community Services, Human Services, Ageing or Disability), with ideally 3+ years industry experience

- Ability to anticipate risk, evaluate impacts and make informed, balanced, confident decisions

- A natural influencer who engages stakeholders and drives alignment and action

- Highly organised, agile and adept at managing competing priorities

- Understands the Strengthened Aged Care Quality Standards, navigating funding and clinical escalation pathways

- Committed to continuous improvement and service excellence

- Balances care outcomes with financial sustainability
