ATSICHS Brisbane is seeking a Social Health Care Coordinator to assess, triage and refer clients to the Social Health team.
The role coordinates intake, screens needs, and delivers case management and social wellbeing supports within a strengths-based framework.
You will work with Psychologists, Counsellors, GPs, Aboriginal Health Workers and Allied Health staff in a multi-disciplinary setting, contributing to culturally secure services and indigenous health outcomes.
