Westmead Rehabilitation Hospital seeks a dedicated Discharge Planner to coordinate protected, seamless patient transitions and personalised discharge plans within a multidisciplinary setting.

You will work closely with Case Managers, families and external providers to ensure timely, patient-centred care journeys and ongoing support post-discharge. AHPRA registration or equivalent is required, with experience across
acute/rehabilitation
settings.
