HCRG Care Group seeks an experienced Clinical Pharmacist (Band 8a) to enhance Hospital at Home services across the BSW network.
You will lead medicines optimisation, provide expert pharmaceutical advice, and contribute to MDT discussions for older, frail patients receiving acute care at home.
The role requires a Masters in Pharmacy, GPhC registration, and at least 2 years in clinical pharmacy, with robust communication skills and IT literacy.
