Job Description
Focus Care Australia in NSW seeks a Care Manager/Care Partner to support participants with effective care management, onboarding, and resource coordination to meet goals within funding and regulatory constraints.

You will screen participants, plan and implement support strategies, monitor outcomes, and ensure transitions. A valid driver's licence and current NDIS clearance are essential; experience in leadership and collaboration with clinical/ally health teams is valued.
