Job Description
Nepean Hospital, part of NSW Health, is seeking a Social Worker to support patients and families across Renal Services. You will assess psychosocial needs and deliver interventions to promote continuity of care and patient safety, working within a multidisciplinary team with a high degree of skilled autonomy.
