Job Description
BlueCare is seeking a Clinical Care Partner to deliver community-based care in Lowood and surrounding areas.
You will spend your day visiting clients, developing and reviewing complex care plans in collaboration with a multidisciplinary team, and ensuring funding outcomes are achieved.
You will manage budgets, monitor care plans, and liaise with clinical teams to respond to changing client needs while promoting client-centred care and outcomes.
