Job Description
RetireAustralia is seeking a Case Manager (Clinical) – RN to join our Support at Home Services team on the Central Coast NSW.
You will coordinate resident care across Tarragal Glen, Wood Glen, Forrester's Beach and Glengara, traveling between villages, with a focus on pre-entry assessments, move‐in support, and ongoing care planning.
The role requires RN registration with AHPRA, a Bachelor of Nursing (or diploma equivalent), and at least 3 years' experience in a similar role.
