About the Role
As a Community Home Care Support Worker, you will provide personalised support to clients living in their own homes, helping them maintain their independence and quality of life.
Your Responsibilities
- Assist with personal care, including showering, dressing, grooming, and mobility support
- Support daily living activities such as meal preparation, medication reminders, shopping, and light household tasks
- Provide companionship and social support
- Assist clients to attend appointments and participate in community activities
- Promote independence, choice, and dignity in all aspects of care
- Maintain accurate documentation and progress notes
- Build positive relationships with clients, families, and healthcare professionals
About You
You are caring, dependable, and passionate about supporting others.
You'll bring
- Certificate III or IV in Individual Support, Aged Care, Home and Community Care, Disability Support, or a related qualification (or currently studying)
- Previous experience in community care, home care, aged care, disability support, or a similar role
- Excellent communication and interpersonal skills
- A compassionate and client-focused approach
- Strong time management and organisational skills
- Ability to work independently and as part of a team
Essential Requirements
- Current National Police Check
- Current First Aid and CPR Certificate
- Current Working with Children Check (or willingness to obtain)
- Valid Australian Driver's Licence
- Reliable and comprehensively insured vehicle
- NDIS Worker Screening Check (desirable)
