About the role
We are seeking a qualified and experienced Community Support Worker to join our home and community care team. The successful candidate will work closely with clients receiving home and community-based care services to assess their individual needs, develop appropriate support strategies, monitor their wellbeing and progress, and coordinate the delivery of services in accordance with individual care plans. The role focuses on promoting clients' independence, wellbeing, dignity and participation in their communities while supporting them to continue living safely and independently in their own homes.
Key responsibilities
- Assess clients' individual needs and circumstances through home visits, consultations and ongoing communication, identifying their social, emotional, health, welfare and daily living support needs.
- Develop and implement individualised care and support plans based on clients' assessed needs, preferences and personal goals.
- Interview and consult with clients and their families or carers to identify difficulties affecting clients' wellbeing, independence and participation in everyday and community activities.
- Monitor and regularly review clients' progress and changing circumstances, evaluate the effectiveness of existing support arrangements, and recommend appropriate adjustments to individual care and support plans.
- Coordinate the delivery of home and community-based support services in accordance with individual care plans and ensure services meet clients' assessed needs and agreed support goals.
- Support clients to maintain social connections and participate in appropriate community activities, promoting independence, confidence and social inclusion.
- Provide clients and their families or carers with information and guidance regarding available home-care services, support arrangements and service options.
- Maintain accurate client records, assessments, case notes, care plans and progress reports and appropriately document changes in clients' circumstances and support requirements.
- Identify concerns relating to clients' safety, wellbeing or ability to live independently at home and communicate relevant concerns to management and responsible members of the care team.
- Contribute to the ongoing review and improvement of home-care programs and service delivery by identifying changing client needs, service gaps and opportunities to improve client outcomes.
About you
- An AQF Certificate IV or higher qualification in Community Services, Aged Care, Disability Services, Individual Support, Social Welfare or a closely related field.
- Relevant experience in community services, home care, aged care, disability services or a related welfare service environment.
- Demonstrated ability to assess client needs and develop and implement individualised care and support plans.
- Experience monitoring client wellbeing, progress and changing support requirements.
- Solid interpersonal and communication skills and the ability to communicate effectively and sensitively with clients, families and carers from diverse backgrounds.
- Good organisational, problem-solving and time-management skills.
- Sound understanding of person-centred care, client dignity, independence, privacy, safeguarding and professional boundaries.
- Ability to prepare and maintain accurate client assessments, case notes, care plans and progress documentation.
- Competency in electronic client-management systems and standard office software.
