Sunrise2Sunrise is seeking an experienced Lead Support Coordinator to provide leadership and practice oversight to our Support Coordination team while maintaining a manageable caseload of participants with complex support needs.
This is a senior hands-on role suited to an experienced Support Coordinator who understands that great Support Coordination is about more than plan utilisation. It requires strong NDIS knowledge, sound judgement, quality documentation, effective stakeholder relationships and a commitment to building participant capacity, choice and control.
As Lead Support Coordinator, you will provide day-to-day guidance and supervision to Support Coordinators, oversee complex matters and escalations, and support consistent, compliant and participant-focused practice across the team.
About the Role
Working closely with senior management, you will:
- provide day-to-day leadership, mentoring and practice guidance to the Support Coordination team
- provide regular supervision, case review and support to Support Coordinators
- manage a caseload of participants with complex or higher-risk support needs
- provide guidance on complex plan implementation, service gaps, changes in circumstances and escalation pathways
- review participant files, progress notes, reports and other documentation to ensure quality, accuracy and compliance
- support the team to prepare high-quality NDIS reports, including evidence for plan reassessments and changes in circumstances
- monitor plan implementation, service agreements, participant outcomes and identified risks
- support effective responses where participants experience service breakdown, safeguarding concerns or other significant changes
- work collaboratively with participants, families, nominees, allied health professionals, Behaviour Support Practitioners, service providers and other stakeholders
- promote participant choice, control, capacity building and supported decision-making
- identify practice or performance concerns and work with management to implement appropriate coaching, development or corrective actions
- contribute to continuous improvement, audits and the ongoing development of Support Coordination practice
- maintain a strong working knowledge of NDIS requirements, Practice Standards, Code of Conduct and relevant pricing and Support Coordination guidance.
About You
You will be an experienced NDIS professional who is confident making decisions, supporting other practitioners and navigating complex participant situations.
You will ideally bring
- demonstrated experience in NDIS Support Coordination, including participants with complex needs
- experience providing leadership, supervision, mentoring or senior practice support to other Support Coordinators
- strong knowledge of the NDIS, Support Coordination requirements and contemporary NDIS practice
- demonstrated ability to manage complex participant matters, risk and service delivery challenges
- excellent written communication skills, particularly progress notes, case documentation and NDIS reporting
- strong organisational skills and the ability to balance leadership responsibilities with participant-facing work
- confidence working with participants, families and multidisciplinary stakeholder groups
- sound qualified judgement and a strong understanding of confidentiality, boundaries and safeguarding
- a commitment to person-centred, strengths-based and rights-based practice.
Qualifications and Screening
Tertiary qualifications in Social Work, Psychology, Allied Health, Community Services, Mental Health, Disability, Human Services or a related discipline are highly regarded. The successful candidate will be required to hold and maintain all mandatory screening and compliance requirements applicable to the role, including:
- Current NDIS Worker Screening Clearance
- Current Working with Children Check – Employee
- Satisfactory National Police Check, and International Police Check where applicable
- Current Australian Driver Licence and access to a reliable vehicle
- Legal entitlement to work in Australia
- Completion of required NDIS Quality and Safeguards Commission worker training, and
- Ongoing compliance with Sunrise2Sunrise mandatory screening, training and professional development requirements.
Experience working with Behaviour Support Plans, positive behaviour support, complex disability and psychosocial support needs will be highly regarded.
