The position of Senior Social Worker (Adolescent Integrated Care) will deliver high level, evidence based, tertiary clinical social work interventions for an adolescent (12-18 years) cohort, where there is identified social complexity and assessed risk of social admission, and/or re-presentation to the Queensland Children’s Hospital (QCH) Emergency Department (ED) due to social factors. Apply high level skills, knowledge, and experience in undertaking complex clinical case management supporting critical follow up and coordination of patients and families where ongoing complexity is identified.
About The Team The Social Work and Welfare Department at QCH is a dedicated, multidisciplinary team committed to supporting the psychosocial well-being of children, young people, and their families. Operating 24 hours a day, 365 days a year, our department provides compassionate, trauma-informed, person-centred care across various clinical settings, including Acute, Medical, Oncology, Child Protection, CHQ at Home, and Post-Trauma services.
About The Role In this role you will coach and support others to deliver for children, young people and families (Leader of Others) in accordance with the Children's Health Queensland Leadership Excellence Framework and the Queensland Public Service and the CHQ values . Your contribution will include:
- Independently triage referrals from Queensland Children's Health Services, including Social Work, Emergency Department, (ED) Medical and Nursing, Child, and Youth Mental Health Service (CYMHS) – Acute Response Team (ART), Child Protection and Forensic Medical Service (CPFMS), Child Development Services (CDS) teams and apply skilled judgement and problem-solving skills, in matters beyond routine practice with infrequent need for direct clinical supervision.
- Deliver high level social work assessment and interventions to patients, their families and stakeholders, utilising trauma informed practice principles and an understanding of adverse childhood experiences,
youth mental health, developmental vulnerabilities, and the biopsychosocial needs relevant to a socially complex, at-risk adolescent cohort.
