Role Description The Support Coordinator is a full-time, in office role based in Gregory Hills, NSW. In this role, the coordinator works directly with participants, families, and stakeholders to understand individual goals and translate NDIS plans into practical supports and services. Day-to-day responsibilities include coordinating and scheduling services, liaising with providers, monitoring progress against goals, and ensuring that supports remain aligned with participants’ changing needs.
The role involves maintaining accurate documentation, preparing reports, and keeping participants informed about available options,
funding, and pathways to increase independence and community participation.
The Support
Coordinator also collaborates with internal clinical and care teams to resolve issues promptly, promote person-centred solutions, and uphold Concept Care’s standards of quality and compliance.
Qualifications
- Experience in support coordination, case management, or similar roles within disability, mental health, community, or aged care services.
- Strong communication and interpersonal skills, including the ability to build trust, advocate for participants, and work collaboratively with families and external providers.
- Knowledge of the NDIS framework, funding pathways, and community support services, with the ability to interpret and implement NDIS plans.
- Organisational and administrative skills, including time management, record-keeping, report writing, and proficiency with common office software.
- Demonstrated commitment to person-centred, culturally sensitive, and trauma-informed practice, with a focus on dignity, choice, and independence.
- Relevant tertiary qualifications in social work, psychology, disability, community services, or a related field (or equivalent experience).
- Ability to work on-site in Gregory Hills, NSW, with a valid driver’s licence and willingness to travel locally as required.
- NDIS Worker Screening Check, Working With Children Check, and other compliance requirements (or eligibility to obtain).
