Support at Home Care Partner | Ulverstone | ASAP Start | Locum assignment for 6 months

We're seeking an experienced Support at Home Care Partner to join a small, supportive team in Ulverstone. This is a full-time locum contract with an inital 6 month duration

What You'll Be Doing

- Managing a caseload of approximately 45 clients
- Coordinating care plans and providing individualised support
- Onboarding new referrals and conducting assessments
- Using MAC (My Aged Care) confidently to manage client journeys
- Working collaboratively within a close-knit team environment

What You'll Bring

- Solid Case Management experience within aged care, community care, NDIS or home care
- Strong MAC platform knowledge and Support at Home knowledge
- Experience onboarding and managing new clients




- A client-first approach with excellent communication skills
- Nursing/Clinical background highly regarded -Div 1 RN or EEN

Role Details

- Location: Ulverstone (office + community) with versatile work options available to reduce travel/office days if coming from Burnie/Launceston/Devonport etc - offering 2-3 days per week remote working, with at least 2 days in the office.
- Start: ASAP
- Hours: Full-time or 4 dpw
- Contract: Locum
