Care n Help is looking for a motivated and organised Support Coordinator to join our growing team.
About the Role
As a Support Coordinator, you will work closely with NDIS participants to help them understand and implement their NDIS plans, connect with suitable services, and build their capacity to achieve their goals.
Key Responsibilities
- Support participants to understand and utilise their NDIS plans
- Connect participants with suitable service providers and community supports
- Coordinate services and communicate with participants, families, providers and other stakeholders
- Monitor participant budgets and service arrangements
- Assist with plan reviews and prepare relevant reports
- Maintain accurate case notes and documentation
- Respond to participant needs and help resolve service-related issues
- Maintain professional boundaries and comply with NDIS requirements
About You
We are looking for someone who:
- Has strong communication and interpersonal skills
- Is organised and able to manage multiple participants and priorities
- Has good written and verbal English skills
- Is confident communicating with participants, families and service providers
- Can work independently and as part of a team
- Has good computer and administrative skills
- Has experience in NDIS, disability, community services, case management or a related field is preferred.
Previous Support Coordination experience is preferred but not essential for the right candidate.
Salary
$65,000-80,000 per year
