Community Based Support (CBS) is a not-for-profit Tasmanian organisation delivering aged care services, to enable people to live safely in their homes and enjoy access to the community as they wish. Our work ensures quality of life is enhanced by providing a range of support services that are tailored to meet individual needs and goals to maximise independence.

We are looking for an enthusiastic and capable Service Delivery Coordinator - Support at Home to join our team and play an important role in delivering high-quality, person-centred care to older Tasmanians.

About the role

As the Service Delivery Coordinator, you will coordinate the delivery of CBS services and supports, with a particular focus on clients receiving Support at Home services.

You will work closely with clients, their families and support networks, staff and external stakeholders to ensure services are responsive, safe, high quality and tailored to individual needs and goals.

You will also provide leadership and support to a team of Support Workers, helping them to succeed in their roles and ensuring our clients receive consistent, respectful and person-centred support.

No two days will be exactly the same. This is a varied role where you will have the opportunity to build solid client relationships, support your team, manage challenges and contribute to the ongoing improvement of our services.

Core Responsibilities





- Coordinate the effective and efficient provision of CBS-delivered supports and services to clients, within budget and according to legislation and funding body guidelines

- Undertake home visits with clients to ensure their needs and requirements continue to be met

- Manage client complaints and concerns with care and discretion, seeking timely and mutually successful resolutions

- Respond to and manage client/staff hazards and incidents, to ensure the safety and wellbeing of CBS clients and staff

- Ensure client files, records and plans are maintained in an appropriate and timely manner

- Lead a team of Support Workers who provide supports to our clients in their homes: recruitment, provision of ongoing feedback and support, lead team meetings, performance oversight and annual performance reviews etc.

- Lead through CBS values with a focus on staff and client health, safety and wellbeing

- Participate in after-hours on-call duties as per the rotating roster shared across the service delivery team

What you bring:

- Relevant qualifications in Community Services, Social/Behavioural Sciences, Individual Support etc. (desired)

- Demonstrated leadership and people management experience - prior management or supervising of a team is essential

- Experience working with a community services environment, assisting clients to access relevant services (including advocating on their behalf) is beneficial

- An understanding of the Aged Care Charter of Rights, Aged Care Principles and Strengthened Aged Care Standards.

- An unrestricted Australian Driver’s License

- Maintain a Working with Vulnerable People Registration, including NDIS Endorsement

- Maintain a Criminal History Check satisfactory to the requirements of the role
