What You'll Be DoingSupporting participants to build meaningful, connected lives beyond their funded supportsHelping participants understand and get the most out of their NDIS plansCoordinating supports efficiently to maximise outcomes and funding utilisationGathering supporting evidence that reflects participants' evolving needs and future funding requirementsIdentifying opportunities across employment, recreation, education and housingAdvocating for participants and liaising with mainstream services to ensure the right supports are in placeYou're the Right Person IfYou have proven experience in a Support Coordination role within the disability sectorYou hold qualifications in Disability, Community Services,



Social Work or a related fieldYou have a strong knowledge of the NDIS and its processesYou're able to manage KPIs and billable hour targets effectivelyYou're a natural connector, excellent with people and confident solving problems with a genuine participant-first approachYou hold a current Driver's Licence, Working with Children Check and NDIS Worker Screening ClearanceWhy Access Support Coordination?
