They are seeking a Registered Nurse to join as a Clinical Care Coordinator, coordinating high quality, person-centred services for clients.This role provides day to day supervision of Support Workers, contributes to workforce planning and roster management, and ensures services are delivered safely and consistently.
Acting as a central point of coordination, the Clinical Care Coordinator works closely with clients, families, support workers, practitioners and external stakeholders to translate client goals and practitioner recommendations into practical, measurable support outcomes.Key ResponsibilitiesProvide clinical leadership and advice to the management and rostering team on client clinical needsFormally review client care plans to ensure compliance with standards of careEnsure individualised,
quality service delivery for clients receiving personal care and supportManage relationships with internal and external stakeholders to support service delivery outcomesLead a team of personal support workers to ensure high quality service deliveryMonitor client funding packages against agreed budgets and address issues as neededPartner and collaborate with clients and stakeholders to deliver appropriate, sustainable servicesRegular local travel by car required, with occasional travel to remote and regional areas by airAbout YouRegistered Nurse with current AHPRA registrationExperience in community services or the disability sector highly regardedAbility to work within a quality management and compliance frameworkStrong organisational skills, able to manage multiple priorities while delivering excellent client outcomesProven experience leading, supervising, mentoring and developing staffAdvanced communication and stakeholder relationship skillsAbility to rapidly acquire knowledge and work within a quality frameworkCurrent Australian driver's licence and reliable vehicleCurrent NDIS Worker Screening Check and Working with Children Check, or willingness to obtainWhat's on OfferComprehensive onboarding and trainingOngoing professional development and growth opportunities opportunitiesSalary packaging to increase take-home payEmployee wellbeing platform for staff and family
