Your Community Health is looking for a Home Care Support Worker to help clients stay independent at home and connected in the community.
You will provide care, social activities and group outings, reporting to the Social Support Coordinator in a casual role in the Northern Suburbs.
You will travel to clients' homes delivering tailored in-home support, fostering independence and meaningful participation.
