We're looking for an experienced Care Partner to join our team in a work-from-home role, supporting older Australians to live independently and comfortably in their own homes.

At the heart of this role are your clients.
n
You'll build genuine relationships, understand what matters to each person and coordinate the services they need to live well at home.
n
You'll also be responsible for bringing new clients on board - with a target of around two current clients per week - completing the associated paperwork and maintaining regular contact with every client through monthly monitoring calls.
n

What you'll be doing
n
You'll be responsible for:
n
n
Building strong, genuine relationships with clients and their families.
n
Understanding client needs, goals and preferences.
n
Developing and coordinating personalised care plans.
n
Coordinating services and providers to make sure clients receive the right support.
n
Advocating for clients and making sure their voice is heard.
n
Managing Home Care Package budgets responsibly.
n
Bringing new clients into the service and completing the required onboarding and documentation.
n
Completing monthly monitoring calls to check that clients are happy and their services continue to meet their needs.
n
Identifying changes in client needs and coordinating appropriate responses.
n
Keeping on top of your documentation, administration and KPIs.
n
Working collaboratively with your manager and wider team.
n
n
The role is about care coordination,
relationships and client outcomes - not providing hands-on nursing care.
n
Who are we looking for?

You could be a:

Support Worker ready to take the next step.
n
Care Coordinator or Case Manager wanting a role with more autonomy.
n
Aged Care professional who understands what great support looks like.
n
Case Manager from another provider who knows how to manage clients, budgets and services.
n
Nurse looking to move into care coordination - provided you're looking for a role that isn't a clinical nursing position.
n
A Certificate IV or higher in aged/community care, care coordination or equivalent experience is required, along with experience in case management,
wellness/reablement
or aged care service delivery.
