The opportunity
You will play apivotal role in supporting the delivery of high-quality,person-centred care through Warrigal Home Services by coordinatingand managing the care needs of clients living in the community.Working closely with the Care Services Manager, Operations Partnerand broader Home Services team, you will ensure services arealigned to individual care plans, client goals and approved supportframeworks.
This role requires someone who is highlyorganised,responsiveandconfident managing competing priorities in adynamicenvironment.You will be responsible for undertakingcomprehensiveassessments,developing and reviewing care plans, and ensuringservices remain responsive to changing client needs.
Your abilityto balance multiple client caseloads while maintaining a strongfocus on outcomes and wellbeing will be essential.
You will collaborate closely with clients, families, serviceproviders, and internal teams including Service Planning andRostering, to ensure services are delivered safely, efficiently andin line with care plans.
Your ability to problem-solve,communicateeffectivelyand respond proactively will be key to minimisingservice disruptions and supporting positive outcomes forclients.
Through proactive care management, regular monitoring andongoing engagement, you will support older people to live safelyand confidently in their own homes and communities.
You will alsocontribute to continuous improvement by identifying opportunitiesto enhance care delivery, promote independence and ensure theeffective use of client funding to achieve optimal outcomes.
Whatyou'llbring
Essential:
Diploma or Certificate IV in Community Services, Aged Care,Case Management or equivalent tertiary qualification.
Demonstrated experience in care coordination, casemanagement or related roles within aged, health or disabilitysectors.
Advanced assessment and care planning skills including theability to identify client needs, risks andsupportrequirements.
Experience working in multidisciplinary careenvironments.




Solid interpersonal and communication skills with theability to build relationships with clients, families andservice providers.
Ability to manage a caseload and prioritise competingdemands
Demonstrated understanding of person-centred care andwellness reablement principles.
Demonstrated ability to network, negotiate andadvocate.
Ability to meet Worker screening requirements
Valid Driver's Licence.
Tertiary qualification in nursing, allied health, communityservices or related
Current registration with the Australian HealthPractitioner Regulation Agency (AHPRA) where applicable
Experience supporting clients with complex care needs.
Knowledge of the Aged Care Act and Support at Home reformchanges
