Job Description
n
Person-Centred Centred and Advocacy
n
Develop and implement goal-focused care plans that promote independence and reablement
n
Regularly review and adapt care in response to client needs
n
Empower clients through consumer-directed care and shared decision-making
n
Leadership & Team Support
n
Support, coach,
and develop care and support staff
n
Assist with performance reviews and team development
n
Foster a positive, collaborative team culture
n
Work closely with schedulers to ensure efficient and effective service delivery
n
Liaise with multidisciplinary teams and external providers
n
Build strong relationships with clients, families, and stakeholders
n
Quality, Compliance & Risk
n
Ensure compliance with Aged Care Quality Standards
n
Maintain accurate documentation and protect client confidentiality
n
Participate in audits, identify risks, and drive continuous improvement
n
Business & Growth Support
n
Contribute to service growth and community engagement initiatives
n
Promote services to local hospitals, ACAT, and community partners
n
What you need to be successful in this role
n
You are a compassionate, organised, and proactive professional who is passionate about delivering high-quality aged care.
n
Demonstrated experience in community aged care (SAH & CHSP)
n
Strong experience in client assessment and care planning
n
Proven leadership and team development skills
n
Excellent time management and problem-solving ability
n
High level of communication and interpersonal skills
n
Experience with electronic care systems and Microsoft 365
n
Sound understanding of Aged Care Quality Standards and compliance
n
Additional Requirements:
n
Current National Police Clearance (within 12months)
n
Ability to participate in an on-call roster
n
Previous case management experience (highly regarded)
n
COVID-19 and Influenza vaccinations (recommended)
n
This role is employed on a salaried contract, and you will be required to participate in an on-call roster one week in each four weeks, including the weekend.When undertaking on-call you will be provided with a day off lieu providing you with flexibility.
