St Vincent's Hospital Sydney is seeking a Social Worker to join the Social Work team (32 hours per week, permanent part time) to coordinate the residential aged care placement service and support timely transfers to RACFs. You will conduct psychosocial assessments, support families, and maintain solid links with RACFs while working with ward teams.

The role offers pro-rata salary from AUD 79,419 to AUD 113,301 plus Super and Salary Packaging, in a renowned inner-city hospital setting.
