Hope & Care Community Services is seeking passionate, compassionate and reliable Support Workers to join our team across Brisbane and surrounding areas.
n
Key responsibilities
n
Provide person-centred support to participants in their home and community.
n
Assist with daily living activities, personal care, personal hygiene and meal preparation as required.
n
Support participants with household duties, shopping and community access.
n
Encourage independence and support participants to maintain meaningful and positive daily routines.
n
Assist participants to participate in community, recreational and social activities.
n
Build respectful and supportive relationships with participants, their families and support networks.
n
Follow individual care plans and provide support according to participant needs and goals.
n
Complete required administrative tasks and documentation.
n
Work collaboratively with other team members and communicate effectively with participants and their support networks.
n
Maintain a secure, clean and respectful environment.
n
About you
n
Current C Class Driver Licence.
n




Access to a registered and roadworthy vehicle.
n
Current NDIS Worker Screening Card.
n
NDIS Worker Orientation Module Certificate.
n
Good communication and interpersonal skills.
n
A genuine commitment to supporting people with disability and promoting their dignity, independence and rights.
n
Experience in disability support, mental health or psychosocial support, youth work, aged care, community services, personal care, or supporting people with complex support needs is highly regarded.
